Why this applies to Saquinavir

Saquinavir falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Passion Flower interacts with.

Theoretically, passion flower might reduce the bioavailability of OATP2B1 and OATP1A2 substrates.
In vitro research shows that the passion flower constituents apigenin and vitexin inhibit OATP2B1 and OATP1A2. This inhibition may be dose-dependent. One specific high-flavonoid passion flower extract (Valverde) seems to inhibit OATP2B1 and OATP1A2, while another extract with a lower flavonoid concentration (Arkocaps) shows less potent inhibition. OATPs are responsible for the uptake of drugs and other compounds into the body; however, the specific activities of OATP2B1 and OATP1A2 are not well characterized.

Here's the short version on combining passion flower with saquinavir (Invirase). There's a theoretical concern that certain passion flower compounds (apigenin and vitexin) may block transporters called OATPs, which help shuttle some drugs into the body. In a test-tube setting, blocking these could lower how much saquinavir gets absorbed. For an HIV medicine, less drug in your system is the worry, since you want steady levels to keep the virus suppressed.

That said, this is based only on lab data, not real patients, and it's rated a minor interaction. I wouldn't lose sleep over it, but because keeping HIV treatment effective matters so much, it's worth a quick chat with your pharmacist or doctor before adding a concentrated passion flower supplement. Don't change your saquinavir on your own.

Saquinavir also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, passion flower might decrease the effects of CYP3A4 substrates.
In vitro research suggests that passion flower can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the gist with saquinavir (Invirase) and passion flower. Your body clears saquinavir using a liver enzyme called CYP3A4. Lab studies hint that passion flower might gently speed up that enzyme, which could mean your body breaks down saquinavir a little faster and leaves less of it in your blood. In theory, lower levels could make your HIV treatment work less well over time, which matters a lot for keeping the virus controlled.

That said, this is based only on test-tube research, and any effect looks weak. I'm not telling you to avoid it, just to loop in your pharmacist or HIV doctor before adding passion flower so they can keep an eye on your viral load.

What Is Saquinavir Used For?

  • Saquinavir is used in combination with ritonavir (Norvir) and other medications to treat human immunodeficiency virus (HIV) infection. Saquinavir is in a class of medications called protease inhibitors. It works by decreasing the amount of HIV in the blood. Although saquinavir does not cure HIV, it may decrease your chance of developing acquired immunodeficiency syndrome (AIDS) and HIV-related illnesses such as serious infections or cancer. Taking these medications along with practicing safer sex and making other lifestyle changes may decrease the risk of transmitting the HIV virus to other people.
