Ibuprofen, Oxycodone falls under ORGANIC ANION TRANSPORTER 3 (OAT3) SUBSTRATES — the group Quercetin interacts with.
In vitro research shows that quercetin is a strong non-competitive inhibitor of OAT3, with half-maximal inhibitory concentration (IC50) values as low as 0.75 mcM. So far, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with this combo pill (the ibuprofen plus oxycodone product sometimes sold as Combunox). Your kidneys use a little transporter called OAT3 to help clear certain drugs out through your urine. In test-tube studies, quercetin blocks that transporter pretty strongly. If the same thing happened in people, the ibuprofen part could hang around a bit longer, which in theory might mean a touch more stomach upset, fluid retention, or kidney strain.
The honest part: this is only lab data right now, and it has never actually been reported in real patients. Everyday food amounts of quercetin are very unlikely to matter. If you're taking high-dose supplements, just mention it to your pharmacist or doctor so they can keep an eye on things.

In vitro research shows that quercetin is a strong non-competitive inhibitor of OAT1, with half-maximal inhibitory concentration (IC50) values less than 10 mcM. So far, this interaction has not been reported in humans.

Combunox combines ibuprofen with oxycodone for short-term pain. The piece that matters here is the ibuprofen half. Your kidneys use a transporter called OAT1 to help clear ibuprofen out of your body. In lab dish studies, quercetin appears to block that transporter. If it does the same thing in people, ibuprofen could leave your system a little slower and build up, which in theory might bump up its usual side effects like stomach upset, heartburn, fluid retention, higher blood pressure, or strain on the kidneys.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro research show that quercetin inhibits CYP2D6. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Combunox combines ibuprofen with oxycodone. The piece to focus on here is the oxycodone, which is partly handled by a liver enzyme called CYP2D6. That enzyme turns some oxycodone into oxymorphone, a more potent active form. In lab (test tube) studies, quercetin can slow CYP2D6 down. In theory, that could shift how much active product your body makes, which might change pain relief or nudge up opioid side effects like drowsiness, dizziness, nausea, or constipation.
Here is the reassuring part: this has only been seen in the lab, never confirmed in real people, and food amounts of quercetin are very unlikely to matter. Concentrated high-dose supplements are the bigger question mark, so check with your pharmacist or doctor before adding one.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
4 of 4Ibuprofen, Oxycodone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

This combination pill pairs ibuprofen (an anti-inflammatory) with oxycodone (an opioid pain reliever). The oxycodone part is the piece quercetin could touch, since your body relies on the CYP3A4 enzyme to help break it down. Quercetin can nudge that enzyme, though the research here is mostly from lab and animal studies, so this is more of a theoretical concern than a proven problem.
If quercetin slows CYP3A4, a bit more oxycodone could linger, which might mean extra drowsiness, dizziness, nausea, constipation, or slowed breathing. Confusingly, one human study hinted quercetin could speed the enzyme up instead, which would weaken pain relief. The everyday amounts in foods are unlikely to matter, but before taking a high-dose supplement, check with your pharmacist or doctor.
