Amitriptyline falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Quercetin interacts with.
In vitro research show that quercetin inhibits CYP2D6. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Your body relies on a liver enzyme called CYP2D6 to help clear amitriptyline. In test-tube studies, quercetin seems to slow that enzyme down. If that happened in real life, amitriptyline could build up a bit more than usual, which might mean stronger side effects like drowsiness, dry mouth, constipation, blurry vision, dizziness when standing, or a faster heartbeat.
Here's the important context though: this has only been seen in lab dishes, not in actual people. So this is more of a theoretical caution than a proven problem. Food amounts of quercetin (apples, onions, berries) are very unlikely to matter. If you're considering a concentrated, high-dose supplement, just run it by your pharmacist or doctor first so they can keep an eye on how you feel.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.

Here's the basic idea: quercetin can slow down a liver enzyme called CYP2C9, which is one of the pathways your body uses to clear amitriptyline. When that pathway is slowed, more of the drug can linger in your system. For an active medication like amitriptyline (it is not a prodrug, so it works directly), that means you might feel its effects more strongly.
In practice, that could show up as extra drowsiness, dry mouth, constipation, dizziness, or a faster heartbeat. I want to be honest, though: this is mostly theoretical and based on studies with other CYP2C9 drugs and high-dose quercetin, not amitriptyline itself. Food amounts of quercetin almost certainly won't matter. Before starting a concentrated supplement, just run it by your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the gist: your body uses a liver enzyme called CYP3A4 to help break down amitriptyline, and quercetin can nudge that enzyme. The tricky part is that quercetin has pulled in two directions in studies. In some lab and animal work it slows the enzyme down, which could let amitriptyline build up a bit and bring out more of its usual side effects like drowsiness, dry mouth, constipation, blurry vision, or a faster heartbeat. In one human study it actually sped the enzyme up, which might make the drug a touch less effective.
The honest truth is that this is mostly theoretical and the evidence is weak, so I wouldn't panic. Quercetin from foods is very unlikely to matter. If you're considering a concentrated supplement, just run it by your pharmacist or doctor first.
