Why this applies to Lovastatin

Lovastatin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the gist: your body uses an enzyme called CYP3A4 to break down lovastatin, and quercetin may interfere with that enzyme. In the lab and in a few animal and human studies, quercetin mostly seemed to block CYP3A4, which could let more lovastatin build up in your blood. With a statin, higher levels usually mean a bigger chance of muscle aches, weakness, or (rarely) more serious muscle problems, plus occasional liver-related effects.

That said, the evidence here is thin and a little mixed, and one human study actually showed the opposite. Food amounts of quercetin (from apples, onions, berries) aren't likely to matter. Concentrated supplements are the bigger question mark. Before adding a quercetin pill, just run it by your pharmacist or doctor so they can keep an eye on things.

Lovastatin also falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP).

Theoretically, concomitant use might increase the effects and adverse effects of OATP substrates.

In vitro evidence shows that quercetin can inhibit organic anion-transporting peptide (OATP) 1B1-mediated uptake of estrone-3-sulfate and pravastatin. Furthermore, clinical research in healthy males shows that intake of quercetin along with pravastatin increases the AUC of pravastatin by 24%, prolongs its half-life by 14%, and decreases its apparent clearance by 18%, suggesting that quercetin modestly inhibits the uptake of pravastatin in hepatic cells.

Here's the gist: quercetin can block a liver transporter called OATP that helps pull certain statins into your liver cells. In a small study with a related statin (pravastatin), quercetin nudged blood levels up by about 24 percent. With lovastatin, the worry is similar, that levels could run a little higher than expected. Lovastatin is active as taken, so more drug hanging around could mean a slightly higher chance of typical statin side effects like muscle aches, soreness, or weakness.

I'd keep this in perspective, though. The evidence here is mostly theoretical and based on another statin, and the effect was modest. Food amounts of quercetin (apples, onions, berries) are very unlikely to matter. If you take concentrated quercetin capsules, just mention it to your pharmacist or doctor so they can keep an eye on things.

About Lovastatin

What Is Lovastatin Used For?

Brand names: Mevacor, Altocor

Lovastatin is used to lower high LDL ("bad") cholesterol and total cholesterol in people whose levels have not improved enough with diet alone. It is also used to slow the buildup of plaque in the arteries (coronary atherosclerosis) in people who already have coronary heart disease.

In adults without existing heart disease but with high cholesterol and low HDL ("good") cholesterol, lovastatin tablets are approved to reduce the risk of heart attack, unstable angina, and the need for procedures to open blocked arteries. Lovastatin tablets are also approved for adolescents aged 10–17 with a specific inherited condition called heterozygous familial hypercholesterolemia (heFH), when diet changes alone haven't been enough.
