Why this applies to Beclomethasone

Beclomethasone falls under P-GLYCOPROTEIN SUBSTRATES — the group Rabdosia Rubescens interacts with.

Rabdosia rubescens might increase levels of p-glycoprotein substrates. In vitro research shows that Rabdosia rubescens extract inhibits p-glycoprotein levels and activity. Drugs that might be affected include some chemotherapeutic agents (doxorubicin, etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), protease inhibitors (amprenavir, indinavir, nelfinavir, saquinavir), H2 antagonists (cimetidine, ranitidine), some calcium channel blockers (diltiazem, verapamil), digoxin, corticosteroids, erythromycin, cisapride (Propulsid), fexofenadine (Allegra), cyclosporine, loperamide (Imodium), quinidine, and others.

Here's the gist for your asthma inhaler. Beclomethasone is a corticosteroid, and your body uses a little pump called P-glycoprotein to help move it out. Lab studies (not human studies) suggest Rabdosia rubescens may slow that pump down. In theory, that could let a bit more beclomethasone hang around, slightly nudging up steroid-type effects.

Two things keep me calm here. First, this is test-tube only evidence, so we really don't know if it happens in people. Second, an inhaled steroid acts mostly right in your airways with low body absorption, so any change is likely small. Beclomethasone isn't a prodrug, so no flip in direction. Still, mention the herb to your pharmacist or doctor before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Beclomethasone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that oridonin, a constituent of Rabdosia rubescens, induces cytochrome P450 3A4 (CYP3A4) enzyme. Also, in an animal model, oridonin induces a gene homologous to CYP3A4. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking Rabdosia rubescens. Some drugs metabolized by CYP3A4 that might be affected are some benzodiazepines such as alprazolam (Xanax), diazepam (Valium), midazolam (Versed), triazolam (Halcion); calcium channel blockers such as amlodipine (Norvasc), diltiazem (Cardizem, Dilacor, others), felodipine (Plendil), verapamil (Calan, Verelan, others); cyclosporine (Neoral, Sandimmune); some HIV antivirals such as indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase); some HMG CoA reductase inhibitors such as atorvastatin (Lipitor), lovastatin (Mevacor); some macrolide antibiotics such as erythromycin and clarithromycin (Biaxin); quinidine; and many others.

Here's the basic idea with this pairing. Your body uses an enzyme called CYP3A4 to help process beclomethasone, and lab studies suggest a compound in Rabdosia rubescens (oridonin) may speed that enzyme up. If it does, your inhaler steroid could get cleared a little faster, which in theory might mean slightly less control of the airway swelling that keeps asthma in check. Practically, that could show up as more wheezing, coughing, or chest tightness.

The honest truth is that this is only test-tube and animal data, and it has never been reported in real people. Beclomethasone is also inhaled and acts mostly right in the lungs, so any whole-body effect is likely small. Still, if you use this herb, mention it to your pharmacist or doctor and watch your breathing.

What Is Beclomethasone Oral Inhalation Used For?

Brand names: Beclovent, Vanceril

Beclomethasone dipropionate HFA inhalation aerosol (brand: Qvar Redihaler) is used for the ongoing, daily management of asthma in adults and children 5 years of age and older. It is a preventive (maintenance) therapy — not a rescue inhaler — meaning it helps control asthma over time rather than treating sudden symptoms.

Important: Qvar Redihaler is not for the relief of sudden asthma attacks or acute bronchospasm. Always keep a separate fast-acting (rescue) inhaler on hand for those moments.
