Here's the honest picture: this is a very theoretical concern. The interaction on file says rauwolscine, a stimulant found in fat-burner and pre-workout products, might lower the seizure threshold and could in theory add to that effect with certain medications. The catch is that the evidence comes only from an animal study, not from people.

With Cosopt (dorzolamide and timolol), this is even less of a worry because it's an eye drop. Only a tiny amount reaches your bloodstream, so the chance of it influencing seizure risk is low. The bigger thing I'd watch with rauwolscine is its own stimulant effect, since it can raise heart rate, bump up blood pressure, and trigger anxiety. If you have any history of seizures, heart issues, or you're unsure, please run it by your pharmacist or doctor first.

Theoretically, rauwolscine might increase levels of drugs metabolized by CYP2D6.
Rauwolscine is structurally related to yohimbine. In vitro research shows that yohimbine inhibits CYP2D6 enzyme activity.

Here's the gist with Cosopt (dorzolamide and timolol). The timolol part is broken down in your body by an enzyme called CYP2D6. Rauwolscine is a cousin of yohimbine, and in lab dish studies yohimbine appears to slow down that same enzyme. So in theory, pairing them could let a bit more timolol build up.

Worth knowing: Cosopt goes in your eye, so the amount that reaches your bloodstream is usually small. Still, if extra timolol gets absorbed, you might notice a slower heart rate, lower blood pressure, or feeling dizzy or tired. This concern is theoretical, based only on lab data, so I don't want to alarm you. Since rauwolscine itself can raise heart rate and blood pressure, just run this combo by your pharmacist or doctor first.

What Is Dorzolamide and Timolol Ophthalmic Used For?

Brand name: Cosopt

Dorzolamide and timolol ophthalmic eye drops are used to reduce elevated intraocular pressure (pressure inside the eye) in people with open-angle glaucoma or ocular hypertension (higher-than-normal eye pressure without signs of glaucoma damage). It is specifically intended for patients who have not responded well enough to beta-blocker eye drops alone.

This combination is available as Cosopt, Cosopt PF, and Dorzolamide Hydrochloride and Timolol Maleate eye drops. Cosopt PF is the preservative-free formulation, which may be preferred for people sensitive to preservatives.
