Why this applies to Dolutegravir
Dolutegravir falls under P-GLYCOPROTEIN SUBSTRATES — the group Rhodiola interacts with.
In vitro research shows that rhodiola inhibits P-glycoprotein. Theoretically, using rhodiola with P-glycoprotein substrates might increase drug levels and potentially increase the risk of adverse effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Rhodiola and dolutegravir (Tivicay). Dolutegravir is carried around your body partly by a transporter called P-glycoprotein, which acts like a little pump. Lab studies (done in test tubes, not in people) suggest Rhodiola may slow that pump down. In theory, that could let a bit more dolutegravir build up, which might nudge up common side effects like headache, trouble sleeping, nausea, or feeling tired.
I want to be honest though: this is a very weak, theoretical concern based only on lab research, so the real risk for most people is likely small. Dolutegravir isn't a prodrug, so it still works the same way. Since keeping HIV treatment steady really matters, just check with your pharmacist or doctor before adding Rhodiola.

Rhodiola is an herb traditionally used to fight fatigue and help the body cope with stress. Some small studies suggest it may modestly reduce fatigue and improve mood, but the evidence is limited and not strong. What Is Dolutegravir Used For?
Brand name: Tivicay
Dolutegravir treats HIV-1 infection. Tivicay and Tivicay PD are used together with other HIV medicines in adults and in children who have not taken an INSTI before.
Tivicay is also used with rilpivirine as a complete regimen. This is for adults whose HIV is already well controlled on a stable regimen for at least 6 months, with no history of treatment failure and no known resistance to either medicine.
