Why this applies to Lapatinib
Lapatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rhodiola interacts with.
In vitro research shows that rhodiola inhibits CYP3A4. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of midazolam, a CYP3A4 substrate.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea with lapatinib (Tykerb): your body relies on a liver enzyme called CYP3A4 to break it down. In a test tube, rhodiola can slow that enzyme. If that were to happen in real life, lapatinib could build up a bit more than expected, which in theory might raise the chance of side effects like diarrhea, nausea, rash, tiredness, or changes in liver or heart function.
That said, I want to keep this in perspective. Lapatinib is the active drug, not a prodrug, so the concern is more (not less) effect. The evidence here is weak and mostly theoretical, and a human study found rhodiola didn't actually slow a similar CYP3A4 drug. Still, since lapatinib is serious cancer therapy, please clear any rhodiola with your oncology team first.

Rhodiola is an herb traditionally used to fight fatigue and help the body cope with stress. Some small studies suggest it may modestly reduce fatigue and improve mood, but the evidence is limited and not strong. What Is Lapatinib Used For?
Brand name: Tykerb
Lapatinib tablets are used in combination with other medicines to treat certain types of advanced or metastatic breast cancer in patients whose tumors overexpress HER2 (a protein that drives tumor growth). It is not used alone.
Specifically, lapatinib tablets are used with capecitabine for HER2-positive advanced or metastatic breast cancer in patients who have already received prior therapy (including certain chemotherapy drugs and trastuzumab). Lapatinib tablets are also used with letrozole for postmenopausal women with hormone receptor-positive, HER2-positive metastatic breast cancer when hormonal therapy is appropriate. Tykerb is the brand name product with these same indicated uses.
