Irinotecan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rooibos interacts with.

Theoretically, taking rooibos with CYP3A4 substrates may increase the effects of CYP3A4 substrates.
In vitro research shows that the methanol extract of rooibos leaves and stems strongly inhibits CYP3A4 enzyme activity.

Here's the idea with rooibos and irinotecan (Camptosar). Your body uses an enzyme called CYP3A4 to help process irinotecan. Lab tests (done in a test tube, not in people) suggest concentrated rooibos extract may block this enzyme. If that happened in real life, more of the drug could linger in your system, which might mean stronger effects and a higher chance of side effects like diarrhea, low blood counts, nausea, and fatigue.

The honest part: this is theoretical and based only on lab data, not human studies, so I don't want to alarm you. Irinotecan is powerful chemotherapy, though, so even small shifts matter. Drinking rooibos as a regular tea is likely far less concerning than concentrated extracts. Please check with your oncology team before combining either.
