Why this applies to Lopinavir, Ritonavir
Lopinavir, Ritonavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rooibos interacts with.
In vitro research shows that the methanol extract of rooibos leaves and stems strongly inhibits CYP3A4 enzyme activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Kaletra (lopinavir plus ritonavir). Both of these drugs are cleared in part by a liver enzyme called CYP3A4, and lab studies hint that concentrated rooibos extract may slow that enzyme down. If that slowing happened in real life, your body would clear the medicine a bit less efficiently, so levels could creep up. In practice that might mean more of the usual side effects, like nausea, diarrhea, belly upset, or changes in blood fats.
I want to be honest, though: this is only test-tube evidence, not something proven in people, so I wouldn't lose sleep over a cup of rooibos tea. Concentrated extract capsules are a different story. Before adding those, please run it by your pharmacist or HIV provider.
