Nirmatrelvir, Ritonavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rooibos interacts with.

Theoretically, taking rooibos with CYP3A4 substrates may increase the effects of CYP3A4 substrates.
In vitro research shows that the methanol extract of rooibos leaves and stems strongly inhibits CYP3A4 enzyme activity.

Here's the gist: Paxlovid relies on the CYP3A4 enzyme, and the ritonavir part is actually there on purpose to slow that enzyme down so the nirmatrelvir stays at a higher level. Lab studies (test tube, not people) suggest that a concentrated rooibos extract might also nudge CYP3A4 in the same blocking direction. In theory, that could let the medication build up a bit more, possibly adding to side effects like altered taste, an upset stomach, or diarrhea.

I want to be honest, though: this is very weak, theoretical evidence, and drinking rooibos as a normal cup of tea is unlikely to cause a problem during your short Paxlovid course. If you're using concentrated rooibos capsules or extracts, run it by your pharmacist or doctor first.
