Lepirudin is a powerful injectable blood thinner, usually given in the hospital to people who can't take heparin. It works by directly blocking thrombin, a key clotting protein. Saw palmetto has been reported to lengthen bleeding time on its own, so the concern is simple: stacking it on top of an already strong anticoagulant could nudge your bleeding risk up a little further. That might show up as easy bruising, nosebleeds, bleeding gums, or, less often, more serious internal bleeding.

That said, this is based on anecdotal reports, not solid studies, so the real-world risk is uncertain. Since lepirudin is given under close medical supervision, just be sure your care team knows about any saw palmetto you take. Don't start or stop anything on your own. Check with your doctor or pharmacist first.
