Acetaminophen, Butalbital, Codeine Phosphate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Schizonepeta interacts with.
Some substrates of CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with this combo pill: the codeine part is actually a prodrug. Your body has to use an enzyme called CYP2D6 to turn codeine into morphine, which is what gives the real pain relief. Lab and animal studies suggest a compound in Schizonepeta may slow that same enzyme down.
So if anything, you might get less pain relief from the codeine, because less of it gets converted to morphine. The acetaminophen and butalbital pieces aren't affected this way. Keep in mind this is based only on early lab and animal work, not on people, so it's quite theoretical. Food-style amounts in a tea are unlikely to matter much. Still, check with your pharmacist or doctor before combining them.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline, zileuton (Zyflo), zolmitriptan (Zomig), and others.

This combination pill mixes acetaminophen with butalbital (a barbiturate) and codeine (an opioid). The interaction on file is based only on animal and lab data, where a compound in Schizonepeta appears to slow down an enzyme called CYP1A2. Honestly, that enzyme plays only a small role here. Codeine actually relies mostly on CYP2D6 to turn into its active form, and butalbital and acetaminophen are handled by other pathways. So this particular interaction is more theoretical than proven for this medicine.
Still, if Schizonepeta did nudge blood levels up a bit, you might notice more drowsiness, dizziness, constipation, or slowed breathing. Since this is a sedating combo already, it's worth a quick chat with your pharmacist or doctor before adding the herb.

CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES

Some substrates of CYP2E1 include acetaminophen, chlorzoxazone (Parafon Forte), ethanol, theophylline, and anesthetics such as enflurane (Ethrane), halothane (Fluothane), isoflurane (Forane), and methoxyflurane (Penthrane).

Here's the piece that matters for your Phrenilin combo: it contains acetaminophen (Tylenol), and acetaminophen is partly handled by a liver enzyme called CYP2E1. In lab and animal studies, a compound in Schizonepeta seems to slow that enzyme down. The theory is that with the enzyme working slower, acetaminophen might hang around a bit longer or shift how it's processed.
Honestly, this is theoretical and based only on animal data, so I wouldn't lose sleep over it. The bigger picture is that any extra acetaminophen exposure could matter for your liver, especially if you also drink alcohol or take other Tylenol-containing products. Keep your total daily acetaminophen in check, and run the Schizonepeta past your pharmacist or doctor before combining them.
