Aspirin, Hydrocodone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Schizonepeta interacts with.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, inhibits cytochrome P450 (CYP) 2D6. Theoretically, schizonepeta might increase the effects and side effects of CYP2D6 substrates.
Some substrates of CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.

This combination pill pairs aspirin with hydrocodone, an opioid pain reliever. Here's the wrinkle: hydrocodone is actually a prodrug. Your liver uses an enzyme called CYP2D6 to turn part of it into hydromorphone, which is a more potent form that helps with pain relief.

Schizonepeta has been shown, in lab and animal studies only, to slow down CYP2D6. In theory, that could mean less hydrocodone gets converted, so you might feel weaker pain relief. The flip side is that more of the parent drug may linger, which makes the direction a bit unpredictable. Keep in mind this evidence is very weak and not proven in people. Still, if you take this herb regularly, mention it to your pharmacist or doctor before combining the two.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, induces cytochrome P450 (CYP) 3A4. Theoretically, schizonepeta might decrease the effects of CYP3A4 substrates.
Some substrates of CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the thing about this combo product: the hydrocodone part is actually a prodrug. Your liver has to convert it into its more active form before it gives much pain relief. Some of that processing involves the same CYP3A4 enzyme that early lab and animal studies suggest Schizonepeta may speed up.

So if anything, revving up that enzyme could shift how your body handles the hydrocodone, and in theory might affect how well your pain is controlled. I want to be honest with you though: this is based only on test-tube and animal data (not human studies), so it's a weak, theoretical concern. The aspirin side isn't really part of this. If you take this medicine for pain and are thinking about Schizonepeta, just run it by your pharmacist or doctor first.
