Galantamine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Schizonepeta interacts with.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, inhibits cytochrome P450 (CYP) 2D6. Theoretically, schizonepeta might increase the effects and side effects of CYP2D6 substrates.
Some substrates of CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.

Here's the simple version: your body uses an enzyme called CYP2D6 to help break down galantamine. Lab and animal studies hint that a compound in Schizonepeta may slow that enzyme down. If clearance slows, more galantamine could linger in your system, which can mean stronger effects and a higher chance of its usual side effects, things like nausea, vomiting, diarrhea, dizziness, a slower heart rate, or loss of appetite.

I want to be honest about the strength of this, though. It's only theoretical, based on test-tube and animal work, not real people, so it may not amount to much in practice. Still, before you pair Schizonepeta with galantamine, check with your pharmacist or doctor so they can weigh it for you.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, induces cytochrome P450 (CYP) 3A4. Theoretically, schizonepeta might decrease the effects of CYP3A4 substrates.
Some substrates of CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the situation with these two. Your body uses an enzyme called CYP3A4 to help clear galantamine. Lab and animal studies hint that Schizonepeta may speed that enzyme up. If that happens in people, galantamine could be broken down a little faster, which in theory might leave less of it working and make it slightly less effective at supporting memory and thinking.

I want to be honest about the evidence: this is only a theoretical concern from animal and test-tube work, not real human studies, so I wouldn't lose sleep over it. Galantamine isn't a prodrug, so we're talking about possibly weaker effect, not more side effects. Still, before adding this herb, run it by your pharmacist or doctor so they can keep an eye on how the medicine is working.

Galantamine — available as extended-release capsules, tablets, and an oral solution — is used to treat mild to moderate dementia caused by Alzheimer's disease. All supported formulations share this indication.

It does not cure or slow the underlying disease, but it can help support cognitive function in people with early to moderate-stage Alzheimer's. Its benefits may lessen as the disease progresses and more brain cells are lost.
