Schizonepeta and Glecaprevir, Pibrentasvir

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Glecaprevir, Pibrentasvir

Glecaprevir, Pibrentasvir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Schizonepeta interacts with.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, induces cytochrome P450 (CYP) 3A4. Theoretically, schizonepeta might decrease the effects of CYP3A4 substrates.
Some substrates of CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Mavyret (glecaprevir and pibrentasvir). Some animal research hints that a compound in Schizonepeta may speed up an enzyme called CYP3A4. Since glecaprevir is partly handled by that enzyme, the worry is that the herb could clear it from your body a little faster, leaving less drug around to fight the hepatitis C virus. With this kind of treatment, you really want steady, full drug levels so the virus gets fully cleared, so anything that lowers the levels is worth respecting.

That said, this is theoretical and based only on lab and animal data, not real people, so the actual risk is uncertain. Please don't stop either one on your own. Just check in with your pharmacist or doctor before adding Schizonepeta while you're on Mavyret.

From our pharmacist’s overview on the Glecaprevir, Pibrentasvir page

“Mavyret (glecaprevir and pibrentasvir) is a short course of pills meant to cure hepatitis C, so I read its supplement list with real caution. Nearly everything on it can shift how much of the drug reaches your blood, mostly through the CYP3A4 enzyme or the P-glycoprotein transporter that handles it.”
