Perampanel falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Schizonepeta interacts with.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, inhibits cytochrome P450 (CYP) 1A2. Theoretically, schizonepeta might increase the effects and side effects of CYP1A2 substrates.
Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline, zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the gist for Fycompa (perampanel). Your liver helps clear this seizure medicine, partly through an enzyme called CYP1A2. Schizonepeta contains a compound that, in lab and animal studies, appears to slow that same enzyme down. In theory, that could let a bit more perampanel build up in your body, which might mean stronger effects and more side effects, things like drowsiness, dizziness, trouble with balance, or mood and irritability changes that perampanel is already known for.

I want to be honest about how thin the evidence is though. This is based only on animal and test-tube research, not real people, so we don't truly know if it matters in humans. Still, since perampanel already causes sedation, it's worth a quick chat with your pharmacist or neurologist before combining the two.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, induces cytochrome P450 (CYP) 3A4. Theoretically, schizonepeta might decrease the effects of CYP3A4 substrates.
Some substrates of CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for Fycompa (perampanel). Your liver relies on an enzyme called CYP3A4 to break this seizure medicine down. In animal studies, a compound in Schizonepeta seems to speed up that enzyme. If that holds true in people, your body could clear perampanel a little faster than usual, which might lower its blood levels and make it less effective at keeping seizures controlled.

I want to be honest about how shaky this is. It comes only from lab and animal research, not from real patients, so it's more of a theoretical concern than a proven problem. Still, because losing seizure control is a big deal, I wouldn't add this herb without first checking with your neurologist or pharmacist. Don't change your perampanel dose on your own.

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
