Pitolisant Hydrochloride falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Schizonepeta interacts with.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, inhibits cytochrome P450 (CYP) 2D6. Theoretically, schizonepeta might increase the effects and side effects of CYP2D6 substrates.
Some substrates of CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.

Here's the basic idea with Wakix (pitolisant) and Schizonepeta. Your body relies on a liver enzyme called CYP2D6 to break pitolisant down and clear it out. One compound in Schizonepeta, called schizonepetin, has slowed that same enzyme in lab and animal studies. If it does the same in people, pitolisant could clear more slowly and build up a bit, which may nudge up its effects and side effects, things like headache, trouble sleeping, nausea, anxiety, or a faster heartbeat.

I'll be honest, this is theoretical and only based on early animal data, so it's far from proven. Pitolisant is not a prodrug, so more drug means stronger effects, not weaker. Before combining them, check with your pharmacist or doctor.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, induces cytochrome P450 (CYP) 3A4. Theoretically, schizonepeta might decrease the effects of CYP3A4 substrates.
Some substrates of CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for pitolisant (Wakix). Your body clears this drug partly through a liver enzyme called CYP3A4. In animal and lab studies, a compound in Schizonepeta seems to speed up that enzyme. If that holds true in people, your system could break pitolisant down a little faster, which may leave less of it working to keep you awake.

In plain terms, the worry is that pitolisant might not control your daytime sleepiness or cataplexy as well as usual. Honestly though, this is only a theoretical concern based on animal data, not human studies, so I wouldn't panic. Just give your pharmacist or doctor a heads-up before combining them, and keep an eye on whether your symptoms creep back.

What Is Pitolisant Used For?

Brand name: Wakix

Wakix (pitolisant) is used to treat two hallmark problems of narcolepsy: excessive daytime sleepiness (feeling overwhelmingly tired and unable to stay awake during the day) and cataplexy (sudden episodes of muscle weakness or loss of muscle control, often triggered by emotion). It is approved for adults and children 6 years of age and older.

Both uses are specific to Wakix tablets taken by mouth. The drug is not approved for sleepiness or cataplexy caused by conditions other than narcolepsy.
