ANTIHYPERTENSIVE DRUGS

Hydrochlorothiazide, Valsartan falls under ANTIHYPERTENSIVE DRUGS — the group Sesame interacts with.
Clinical research shows that replacing other cooking oil with sesame oil can lower systolic blood pressure (SBP) and diastolic blood pressure (DBP) in patients with or without hypertension. There is also some evidence that sesame oil has additive effects in patients also taking atenolol, nifedipine, and/or hydrochlorothiazide. In patients using nifedipine, using a blend of sesame oil and rice bran oil for cooking reduces both SBP and DBP more than nifedipine alone.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Diovan HCT pairs two blood pressure lowerers: valsartan, which relaxes your vessels, and hydrochlorothiazide, a water pill that helps your body shed extra fluid and salt. The thing to know with sesame is that it isn't changing how your body breaks down these medicines. Instead, sesame oil seems to have a mild blood-pressure-lowering effect of its own, so using it regularly may simply add to what your medication is already doing.
In plain terms, your numbers could dip a bit lower than expected. If they go too low, you might feel dizzy, lightheaded, tired, or woozy when standing up. This is theory-based and likely modest, and a little sesame in cooking is unlikely to matter. Still, if you use a lot of sesame oil, mention it to your pharmacist or doctor so they can keep an eye on your readings.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

In vitro, sesame inhibits CYP2C9. However, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version on combining sesame with Diovan HCT (valsartan plus hydrochlorothiazide). In lab studies, compounds in sesame seem to slow down a liver enzyme called CYP2C9. Valsartan is handled in part by this enzyme, so in theory sesame could let a little more of it linger in your body. If that happened, you might notice stronger blood-pressure effects, like feeling dizzy, lightheaded, or extra tired, especially when standing up.
That said, this is purely theoretical. It has only been seen in test tubes, never in real people, and normal food amounts of sesame on a bun or in tahini are very unlikely to cause trouble. Concentrated sesame supplements are less studied. If you take those daily, mention it to your pharmacist or doctor, but there's no need to avoid sesame in your meals.
