Sesame and Losartan Potassium

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Sesame interacts with whole categories of medication. Losartan Potassium falls into 2 of those categories, so it can interact with Sesame in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

ANTIHYPERTENSIVE DRUGS

1 of 2

Why this applies to Losartan Potassium

Losartan Potassium falls under ANTIHYPERTENSIVE DRUGS — the group Sesame interacts with.

Taking sesame oil with antihypertensive drugs might increase the risk of hypotension.
Clinical research shows that replacing other cooking oil with sesame oil can lower systolic blood pressure (SBP) and diastolic blood pressure (DBP) in patients with or without hypertension. There is also some evidence that sesame oil has additive effects in patients also taking atenolol, nifedipine, and/or hydrochlorothiazide. In patients using nifedipine, using a blend of sesame oil and rice bran oil for cooking reduces both SBP and DBP more than nifedipine alone.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: sesame oil itself appears to gently nudge blood pressure downward. So if you're already taking losartan (Cozaar) to lower your blood pressure, regularly cooking with a lot of sesame oil could add to that effect and push your numbers a little lower than expected.

If your pressure drops too far, you might feel dizzy, lightheaded, tired, or wobbly when standing up quickly. To be clear, this is a theoretical, mild concern based on how sesame behaves, not a dangerous combination. A drizzle of sesame oil for flavor is unlikely to matter. Larger, regular cooking amounts are what the research looked at. If you notice low readings or symptoms, just check in with your pharmacist or doctor before changing anything.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

2 of 2

Losartan Potassium also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, sesame might increase the levels and clinical effects of CYP2C9 substrates.
In vitro, sesame inhibits CYP2C9. However, this interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's a wrinkle worth knowing: losartan is actually a prodrug. Your body uses an enzyme called CYP2C9 to convert it into its more powerful active form, which does most of the blood-pressure lowering. In lab dish studies, compounds in sesame appear to slow down that same enzyme. If that happened in real life, it could mean a little less of losartan's active form gets made, which might soften how well it controls your blood pressure.

The honest truth is this is purely theoretical right now. It comes from test-tube data, not from real people, and the everyday sesame on your bagel or in your stir-fry almost certainly won't move the needle. Concentrated sesame supplements are less studied. If you take those regularly, it's worth a quick chat with your pharmacist or doctor, and keep an eye on your home blood pressure readings.

About Sesame

What is Sesame?

  • Heart and cholesterol health
  • Blood pressure support
  • Source of healthy fats and minerals
  • Skin and hair care (topical oil)
  • Antioxidant support
  • Cooking and nutrition

Sesame is a nutritious seed and oil widely used in cooking and traditional medicine, and it provides healthy fats, fiber, and antioxidant compounds called lignans. Some early research suggests sesame may modestly help with cholesterol and blood pressure, but the evidence is limited. Sesame is also a common, sometimes serious, food allergen, so people with seed allergies should be careful.

About Losartan

What Is Losartan Used For?

Brand name: Cozaar

Losartan tablets — available as branded Cozaar and several generic products — are used to treat high blood pressure (hypertension) in adults and children 6 years and older. Lowering blood pressure reduces the risk of strokes and heart attacks. Losartan tablets are also used to reduce the risk of stroke in people with high blood pressure who also have left ventricular hypertrophy (an enlarged left side of the heart). Note: evidence suggests this stroke-risk benefit may not apply to Black patients.

Losartan tablets are additionally approved to protect the kidneys in people with type 2 diabetes, high blood pressure, and signs of kidney damage (diabetic nephropathy). Arbli, the oral suspension form, shares these same approved uses.
