Here's the thing to keep in mind with this pairing. Alfentanil is a strong, fast-acting opioid given in hospital settings during procedures, and it already pushes the brain and breathing toward sedation. The concern on file is that shepherd's purse may have its own mild calming or sedative-type effect, so stacking the two could, in theory, add up to more drowsiness, deeper sleepiness, or slowed breathing than expected.

That said, this is a theoretical worry based on how these substances behave, not on real patient studies, so I don't want to alarm you. Shepherd's purse doesn't speed up or block alfentanil's breakdown here, so the risk is simply overlap of effects. Since alfentanil is given under close monitoring, mention any herbal supplements to your anesthesia team or pharmacist beforehand.
