CNS DEPRESSANTS
Why this applies to Fentanyl
Fentanyl falls under CNS DEPRESSANTS — the group Shepherd's Purse interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with Shepherd's Purse and Fentanyl. Fentanyl is a strong opioid that calms your brain and nervous system, which is exactly what makes it work for pain but also why it can cause drowsiness, slowed breathing, and a foggy head. Some traditional herbs, including shepherd's purse, are thought to have mild calming or sedative-type effects of their own.
So the concern is simply additive sleepiness. In theory, stacking the two could leave you more drowsy or slow your breathing a bit more than fentanyl alone. I'll be honest though, this is based on how the herb may behave, not on solid human studies, so it's a low-confidence caution. With a medicine as powerful as fentanyl, please run any herbal supplement past your doctor or pharmacist first.

From our pharmacist’s overview on the Fentanyl page
“There is very little room for error with fentanyl (Duragesic, Sublimaze, Actiq, and other forms), a powerful opioid where a modest rise in blood level or an extra layer of sedation can become dangerous, so this interaction list deserves real respect. Most of the risk falls into two patterns.”

About Shepherd's Purse
What is Shepherd's Purse?
- Heavy or prolonged menstrual bleeding
- Minor bleeding (nosebleeds, wounds)
- Mild diuretic
- Digestive complaints
- After-childbirth bleeding (traditional)
Shepherd's purse is a traditional herb most often used to help slow heavy menstrual bleeding and other minor bleeding, but solid human research is very limited. Because it may affect blood clotting, blood pressure, and the uterus, it should be avoided during pregnancy and used cautiously under professional guidance.
About Fentanyl
What Is Fentanyl Used For?
Brand names: Sublimaze, Duragesic, Fentora, Lazanda, Abstral, Subsys
Fentanyl transdermal system is prescribed for severe, persistent pain that cannot be controlled with other pain medicines, including shorter-acting opioids. It is only for people who are already opioid-tolerant — meaning they've been taking a significant daily opioid dose for at least a week.
It is not for short-term pain, post-surgical pain, mild pain, or use on an as-needed basis. Because of serious risks including addiction, overdose, and death, it is reserved for situations where other treatment options have failed or are not appropriate.
