Diethylstilbestrol, Methyltestosterone falls under HEPATOTOXIC DRUGS — the group Spearmint interacts with.

Theoretically, high doses of spearmint might increase the risk of liver damage when taken with hepatotoxic drugs.
Animal research suggests that drinking spearmint tea for 30 days can increase markers of liver damage, including aspartate aminotransferase (AST) and alanine aminotransferase (ALT), and cause liver degeneration and necrosis, in a dose-dependent manner. This effect has not been reported in humans.

Here's the gist: the combination pill Tylosterone (diethylstilbestrol plus methyltestosterone) can already put some stress on your liver. Methyltestosterone in particular is well known for occasionally raising liver enzymes or causing other liver trouble. The concern with spearmint is that very high, concentrated doses might add a little extra burden on top of that.

I want to be honest about how soft this evidence is. It comes only from animal studies where rodents drank large amounts of spearmint tea, and nothing like it has been seen in people. Ordinary culinary amounts (a cup of tea or mint in your food) are very unlikely to be a problem. If you're considering concentrated spearmint capsules while taking this medication, just run it by your pharmacist or doctor first so they can keep an eye on your liver labs.
