Acetaminophen, Chlorpheniramine, Phenylpropanolamine, Opium falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Sulforaphane interacts with.

Theoretically, sulforaphane might alter the levels and clinical effects of CYP1A2 substrates.
Animal and in vitro research shows that sulforaphane inhibits CYP1A2 enzyme activity. However, its precursor glucoraphanin also appears to increase the expression of the CYP1A2 enzyme, which could lead to increased metabolism of CYP1A2 substrates. These effects have not been reported in humans.

This combo product is a mix of a pain reliever (acetaminophen), an antihistamine (chlorpheniramine), a decongestant (phenylpropanolamine), and a mild opioid (opium). The piece tied to sulforaphane is the CYP1A2 enzyme, which plays a small role in how the body handles some of these ingredients. Here's the honest part: in lab dishes and animals, sulforaphane has been shown to slow this enzyme, while its precursor may speed it up. The two effects pull in opposite directions, and neither has actually been seen in people.

So in practice, eating broccoli or normal cruciferous veggies won't meaningfully change how this medicine works. Concentrated sulforaphane supplements are less studied, so if you take one, just mention it to your pharmacist or doctor to be safe.

This combination product contains a few different ingredients, and one of them, the opium component (an opioid pain and cough ingredient), leans on the CYP3A4 enzyme to be cleared from your body. Here's the theory: lab and animal studies suggest sulforaphane may slow that enzyme down. If that held true in people, the opioid could linger a bit longer and hit harder, meaning more drowsiness, dizziness, constipation, or slowed breathing. The antihistamine (chlorpheniramine) could add to that sleepy feeling too.

I want to be honest with you though: this has only been seen in test tubes and animals, never confirmed in humans, so it's more of a maybe than a sure thing. Food amounts of broccoli are very unlikely to matter. If you take a concentrated sulforaphane supplement, just check with your pharmacist or doctor first.

This combination product contains acetaminophen (Tylenol), and that's the piece that connects to sulforaphane here. A liver enzyme called CYP2E1 helps break down acetaminophen, and in lab dish studies sulforaphane appears to slow that enzyme down. In theory, that could mean a bit more acetaminophen stays around. The good news is this matters in two ways: it might lower the small amount of toxic liver byproduct acetaminophen normally makes, but the bigger real-world concern is just not stacking too much acetaminophen overall.

Keep in mind this is only test-tube evidence and has never been seen in actual people. Broccoli on your plate is no worry at all. If you take a concentrated sulforaphane capsule and use this medicine regularly, check with your pharmacist, and never exceed the labeled acetaminophen dose.
