Why this applies to Acetaminophen, Chlorzoxazone

Acetaminophen, Chlorzoxazone falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Sulforaphane interacts with.

Theoretically, sulforaphane might alter the levels and clinical effects of CYP1A2 substrates.
Animal and in vitro research shows that sulforaphane inhibits CYP1A2 enzyme activity. However, its precursor glucoraphanin also appears to increase the expression of the CYP1A2 enzyme, which could lead to increased metabolism of CYP1A2 substrates. These effects have not been reported in humans.

Here's the short version on combining sulforaphane (the broccoli compound) with this combo pill that contains acetaminophen plus the muscle relaxer chlorzoxazone. The interaction we have on file is purely theoretical, based only on lab and animal studies, with nothing reported in actual people. In those studies, sulforaphane appears to block an enzyme called CYP1A2, which helps process chlorzoxazone. If that happened in humans, chlorzoxazone could build up a bit and lead to more drowsiness, dizziness, or stomach upset.

To muddy it further, a related broccoli compound may actually speed up that same enzyme, so the direction isn't clear. Eating broccoli at dinner is not a concern. If you take a concentrated sulforaphane supplement, just run it by your pharmacist or doctor first.

Here's the gist for this combo pill, which pairs a muscle relaxant (chlorzoxazone) with acetaminophen. Lab studies suggest sulforaphane can slow down an enzyme called CYP2E1, which is one of the pathways your body uses to clear chlorzoxazone. If that enzyme is dialed back, chlorzoxazone could build up a bit more than usual, which in theory might mean stronger effects like extra drowsiness, dizziness, or lightheadedness.

The important caveat: this is only test-tube evidence, and it has never actually been reported in real people. Everyday broccoli on your plate is very unlikely to matter here. A concentrated sulforaphane supplement is the bigger question mark, so if you take one regularly, just run it by your pharmacist or doctor before combining.
