Why this applies to Atazanavir

Atazanavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sulforaphane interacts with.

Theoretically, sulforaphane might increase the levels and effects of CYP3A4 substrates.
Animal and in vitro research shows that sulforaphane inhibits CYP3A4 enzyme activity and downregulates its expression. This interaction has not been reported in humans.

Here's the gist: your body uses an enzyme called CYP3A4 to clear atazanavir from your system. In lab and animal studies, sulforaphane appears to slow that enzyme down a bit. If that happened in people, atazanavir could build up to higher levels, which might mean more of its typical side effects, like nausea, headache, or the harmless yellowing of the skin and eyes (jaundice) that this drug is known for.

The honest part: this is purely theoretical right now. It comes from test-tube and animal data, and it has never been reported in actual patients. Everyday broccoli and cruciferous veggies are not a concern. Before starting a concentrated sulforaphane supplement, just run it by your pharmacist or HIV provider so they can keep an eye on things.

About Atazanavir

What Is Atazanavir Used For?

Brand name: Reyataz

Atazanavir capsules are used together with other antiretroviral medicines to treat HIV-1 infection in adults and in children 6 years and older who weigh at least 15 kg (about 33 lbs).

Reyataz — available as capsules and as an oral powder — is also approved for HIV-1 treatment in adults and in children as young as 3 months who weigh at least 5 kg (about 11 lbs). Reyataz oral powder must always be taken with ritonavir and food, and is not for use in children weighing less than 5 kg.
