Why this applies to Bedaquiline

Bedaquiline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sulforaphane interacts with.

Theoretically, sulforaphane might increase the levels and effects of CYP3A4 substrates.
Animal and in vitro research shows that sulforaphane inhibits CYP3A4 enzyme activity and downregulates its expression. This interaction has not been reported in humans.

Here's the basic idea with these two together. Your body relies on an enzyme called CYP3A4 to clear bedaquiline out of your system. In lab and animal studies, sulforaphane seems to slow that enzyme down. If that happened in people, bedaquiline could build up a bit higher than expected, which might mean more side effects, like nausea, headache, joint pain, or changes in your heart's rhythm (a longer QT interval) that doctors already watch for with this drug.

I want to be honest about how shaky this is. This is only a theoretical concern from test-tube and animal work, and it hasn't been seen in humans. Eating broccoli is fine. If you're considering a concentrated sulforaphane supplement while on TB treatment, please run it by your pharmacist or doctor first.

What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
