Ivacaftor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sulforaphane interacts with.

Theoretically, sulforaphane might increase the levels and effects of CYP3A4 substrates.
Animal and in vitro research shows that sulforaphane inhibits CYP3A4 enzyme activity and downregulates its expression. This interaction has not been reported in humans.

Here's the situation with Kalydeco (ivacaftor): your body relies heavily on an enzyme called CYP3A4 to break it down and clear it out. In lab and animal studies, sulforaphane (the compound concentrated in broccoli sprout extracts) appears to slow that same enzyme down. If that held true in people, ivacaftor could clear more slowly and build up a bit, which might mean stronger effects and a higher chance of side effects like headache, dizziness, belly pain, diarrhea, or rash.

The honest part: this is theoretical and only seen in test tubes and animals, never confirmed in humans. Everyday broccoli on your plate is very unlikely to matter. A concentrated supplement is the real question, so check with your pharmacist or CF team before adding one.

What Is Ivacaftor Used For?

Brand name: Kalydeco

Kalydeco (ivacaftor) is used to treat cystic fibrosis (CF) in patients aged 1 month and older who have at least one mutation in the CFTR gene that responds to ivacaftor. A specific genetic test is used to confirm whether a patient's mutation qualifies.

There is a long list of CFTR mutations for which Kalydeco is approved — including G551D, R117H, F508C, S549N, and many others. If your or your child's mutation is unknown, an FDA-cleared CF mutation test should be done first to confirm eligibility.
