Letrozole, Ribociclib Succinate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sulforaphane interacts with.

Theoretically, sulforaphane might increase the levels and effects of CYP3A4 substrates.
Animal and in vitro research shows that sulforaphane inhibits CYP3A4 enzyme activity and downregulates its expression. This interaction has not been reported in humans.

Your Kisqali plus Femara combo is partly cleared by a liver enzyme called CYP3A4. In test tube and animal studies, sulforaphane seems to slow that enzyme down. If the same thing happened in people, your body might clear these medicines a little slower, letting levels creep up. With ribociclib especially, higher levels could mean more nausea, fatigue, low blood counts, or heart rhythm changes (a longer QT interval). Letrozole side effects like hot flashes or joint aches could also feel stronger.

Here's the reassuring part: this is theoretical and has not been seen in humans. Eating broccoli at dinner is very unlikely to matter. A concentrated sulforaphane supplement is a different story, so please run that by your oncology team or pharmacist first.
