Sirolimus (albumin bound) falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sulforaphane interacts with.

Theoretically, sulforaphane might increase the levels and effects of CYP3A4 substrates.
Animal and in vitro research shows that sulforaphane inhibits CYP3A4 enzyme activity and downregulates its expression. This interaction has not been reported in humans.

Here's the gist: your body uses an enzyme called CYP3A4 to break down sirolimus (Fyarro). In lab and animal studies, sulforaphane appears to slow down that same enzyme. If that held true in people, sirolimus could clear more slowly and build up to higher levels in your blood. Since sirolimus has a narrow safety window, even modest increases can matter, potentially meaning more side effects like mouth sores, low blood counts, raised blood sugar or cholesterol, swelling, or a higher infection risk.

The reassuring part: this is theoretical only, seen in test tubes and animals, never reported in humans, and the broccoli on your dinner plate isn't the concern. Concentrated supplements are a different story, so check with your oncology team or pharmacist before starting one.
