Why this applies to Aliskiren

Aliskiren falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sweet Annie interacts with.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the gist for Aliskiren (Tekturna). Your body uses an enzyme called CYP3A4 to help process this blood pressure medicine. Sweet Annie may nudge that enzyme one way or the other, so in theory it could shift how much aliskiren stays active in your blood. If levels drop, your pressure may not be controlled as well; if they climb, you might notice more dizziness, low blood pressure, or higher potassium.

I want to be honest about the evidence here: this is only based on lab studies, and the results actually pointed in both directions, so nobody knows the real effect in people yet. Aliskiren also isn't a prodrug, so it doesn't flip the usual logic. Don't change anything on your own. Just give your pharmacist or doctor a heads-up before combining the two.

Aliskiren and Tekturna tablets are both approved to treat high blood pressure (hypertension) in adults and in children who are at least 6 years old and weigh 50 kg (about 110 lbs) or more. Lowering high blood pressure reduces the risk of serious events like strokes and heart attacks.
