Why this applies to Baricitinib

Baricitinib falls under HEPATOTOXIC DRUGS — the group Sweet Clover interacts with.

Theoretically, concomitant use of sweet clover with hepatoxic drugs might increase the risk of hepatotoxicity.
Orally, large amounts of sweet clover can cause transient liver injury in susceptible individuals, possibly due to coumarin content. In one case report, a patient with multiple sclerosis developed jaundiced palms and a slight elevation in serum alanine aminotransferase (ALT) after taking sweet clover containing coumarin 10 mg daily for 3 years. Two weeks after starting therapy with subcutaneous interferon (IFN)-beta 1b, aspartate transaminase (AST) and ALT levels increased to 7 and 17 times the upper limit of normal, respectively. Levels normalized after discontinuation of both products. Subsequent use of IFN-beta 1a alone, without sweet clover, resulted in only a very slight increase in AST.

Here's the thing to know with baricitinib (Olumiant): it can occasionally raise your liver enzymes on its own, so anything else that stresses the liver is worth a second look. Sweet clover contains coumarin, and in large or concentrated supplement doses it has been linked to mild, reversible liver irritation in some people. Put the two together and the worry is simply an added burden on the liver, not a dramatic chemical reaction.

To be clear, this is a theoretical concern based on how each acts, not strong proof. Baricitinib isn't a prodrug here, so this isn't about changing how well it works. Still, since your doctor already checks liver labs on this medicine, please mention any sweet clover use to your pharmacist or doctor before combining them.

What Is Baricitinib Used For?

Brand name: Olumiant

Olumiant (baricitinib) tablets are approved for three conditions in adults. It treats moderately to severely active rheumatoid arthritis in people who haven't responded well enough to one or more TNF blocker medications. It also treats severe alopecia areata (significant hair loss from an autoimmune condition).

Olumiant is additionally approved to treat COVID-19 in hospitalized adults who need supplemental oxygen, mechanical ventilation, or ECMO (a machine that does the work of the heart and lungs). Each use has specific limitations — for example, Olumiant should not be combined with other JAK inhibitors or certain strong immunosuppressants for rheumatoid arthritis or alopecia areata.
