Acetaminophen, Butalbital, Codeine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP2D6 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP2D6 enzymes and reduces CYP2D6 substrate metabolism.

Here's the wrinkle with this combo. The codeine part of your medicine is actually a prodrug, meaning your body has to convert it using an enzyme called CYP2D6 before it becomes the pain-relieving form (morphine). Lab studies hint that Terminalia arjuna may slow that same enzyme down. If that happened in a real person, you'd expect less codeine to get switched on, so you might notice weaker pain relief rather than more side effects.

That said, this is based only on test-tube research, not on actual patients, so it's quite theoretical. The acetaminophen and butalbital pieces aren't really driven by CYP2D6. If you want to use both, just run it by your pharmacist or doctor first so they can keep an eye on whether your pain is well controlled.
