Budesonide, Formoterol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Terminalia interacts with.
In vitro research shows that Terminalia arjuna extract inhibits CYP2D6 enzymes and reduces CYP2D6 substrate metabolism.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest version: this flag comes from the idea that Terminalia arjuna might block a liver enzyme called CYP2D6, which could let drugs cleared by that enzyme build up a bit. With Symbicort, the formoterol piece does have some CYP2D6 involvement, so in theory higher levels could mean more of formoterol's typical effects, like a faster or pounding heartbeat, jitteriness, shakiness, or a headache. Budesonide itself is mainly handled by a different enzyme (CYP3A4), so it's less affected here.
Keep in mind this is only test-tube evidence, not something proven in people, so the real-world risk is likely small. Still, since Symbicort already affects the heart, it's worth a quick chat with your pharmacist or doctor before adding Terminalia, especially if you have a heart condition.

Here's the honest scoop on combining Terminalia with your Symbicort inhaler. The concern on file is that Terminalia arjuna may block a liver enzyme called CYP2C9, which can theoretically let certain drugs build up a little. If that happened with the budesonide part (the steroid), you might see slightly more steroid side effects over time, like a hoarse voice, oral thrush, or with heavy long-term use, things like bruising or blood sugar changes.
That said, I want to keep this in perspective. This warning comes only from lab (test tube) research, not from real people, so it's very theoretical. Budesonide isn't a prodrug, and inhaled doses are quite small. I wouldn't lose sleep over it, but do mention the Terminalia to your pharmacist or doctor so they can keep an eye on you.
