Terminalia and Dalteparin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTICOAGULANT/ANTIPLATELET DRUGS

Why this applies to Dalteparin

Dalteparin falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Terminalia interacts with.

Theoretically, concomitant use of Terminalia arjuna with anticoagulant or antiplatelet drugs may increase the risk of bleeding in some patients.
In vitro, Terminalia arjuna bark extract inhibits platelet aggregation, decreases platelet activation, and shows antithrombotic properties.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Dalteparin is an injectable blood thinner, so it already lowers your blood's ability to clot. Here's the concern with Terminalia: in lab studies, arjuna bark extract made platelets stickier to clump and showed clot-fighting activity of its own. Stack that on top of dalteparin and, in theory, you could tip a little further toward bleeding.

I want to be honest about the evidence though. This is based only on test-tube and animal work, not real patients, so it's more of a caution than a proven problem. Still, watch for things like easy bruising, bleeding gums, nosebleeds, pink urine, or dark stools. Please don't start or stop anything on your own. Loop in your doctor or pharmacist first, especially since you're on dalteparin for a real clotting reason.

From our pharmacist’s overview on the Dalteparin page

“Dalteparin (Fragmin) is an injectable blood thinner used to prevent and treat dangerous clots, which is exactly why this list warrants real vigilance, and nearly all 218 ingredients on it thin the blood too. They are herbs and extracts with clot-inhibiting or antiplatelet effects of their own, so stacking them on dalteparin can push bl…”

  • Immune support

Terminalia is a group of traditional Ayurvedic tree species (most notably Terminalia arjuna) used for heart, digestive, and general wellness purposes. About Dalteparin Injection

What Is Dalteparin Injection Used For?

Brand name: Fragmin

Fragmin is used to prevent clot-related problems in several situations. These include unstable angina and non-Q-wave heart attack (taken with aspirin), and deep vein thrombosis (DVT) after hip or abdominal surgery or during an acute illness that severely limits movement.

It is also used to treat symptomatic venous thromboembolism (VTE, a clot in a vein) and lower the chance it comes back. This applies to adults with cancer and to children from birth (gestational age at least 35 weeks). Fragmin is not indicated for the acute treatment of VTE.
