Enfortumab Vedotin-Ejfv falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Here's the idea behind this one. Lab studies suggest Terminalia arjuna may block an enzyme called CYP3A4. Padcev (enfortumab vedotin) carries a small chemotherapy payload that is partly cleared by that same enzyme. If the enzyme is slowed down, in theory more of that drug could linger in your body, which might mean stronger effects and a higher chance of side effects like numbness or tingling in the hands and feet (neuropathy), skin rash, high blood sugar, fatigue, or eye problems.

That said, this is a theoretical concern based only on test-tube work, not on real patients. We don't have human data showing it actually happens. Still, since Padcev is a serious cancer infusion, please don't add Terminalia without first checking with your oncology team or pharmacist.

What Is Enfortumab vedotin-ejfv Injection Used For?

Brand name: Padcev

  • Enfortumab vedotin-ejfv injection is used to treat urothelial cancer (cancer of the lining of the bladder and other parts of the urinary tract) that has spread to nearby tissues or other parts of the body or cannot be removed by surgery, and has worsened after treatment with other chemotherapy medications or if these chemotherapy medications cannot be used for treatment. Enfortumab vedotin-ejfv injection is in a class of medications called monoclonal antibodies. It works by helping your immune system to slow or stop the growth of cancer cells.
