Fostamatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Here's the gist of it. Your body uses an enzyme called CYP3A4 to help clear fostamatinib (after it becomes its active form, R406). Lab studies suggest that Terminalia arjuna may slow down that enzyme. If clearance slows, more of the active drug can hang around, which could nudge up its effects and side effects, things like diarrhea, high blood pressure, nausea, or changes in liver enzymes.

I want to be honest about the strength here: this is based only on test-tube research, not on people, so it's theoretical. Food-level amounts are unlikely to matter much, but concentrated supplement doses are the bigger question. Please check with your pharmacist or doctor before pairing these, especially since they may want to watch your blood pressure and labs.

What Is Fostamatinib Used For?

Brand name: Tavalisse

Tavalisse (fostamatinib) is used to treat thrombocytopenia — dangerously low platelet counts — in adults with chronic immune thrombocytopenia (ITP). It is specifically for people whose platelet counts have not improved enough with at least one earlier ITP treatment.

The goal is to raise platelet counts high enough to reduce the risk of serious bleeding. If platelet counts do not respond sufficiently after 12 weeks, the medication is typically stopped.
