Macitentan, Tadalafil falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Terminalia interacts with.
In vitro research shows that Terminalia arjuna extract inhibits CYP2C9 enzymes and reduces CYP2C9 substrate metabolism.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Opsynvi combines macitentan and tadalafil, and part of how your body clears these medicines runs through a liver enzyme called CYP2C9. The idea here is that Terminalia arjuna may slow that enzyme down. If it does, these drugs could linger a bit longer in your system and their effects, including blood-pressure lowering, could be nudged upward. In real life that might show up as more headache, flushing, stuffy nose, dizziness, or lightheadedness when you stand up.
I want to keep this in perspective, though. This concern comes only from lab (test tube) studies, not from people, so it is theoretical at this point. Still, since Opsynvi already affects blood pressure, it is worth a quick chat with your pharmacist or doctor before adding Terminalia.

Opsynvi combines two medicines, macitentan and tadalafil, and both are cleared from your body by a liver enzyme called CYP3A4. Terminalia arjuna, in lab (test tube) studies only, seems to slow that enzyme down. If it did the same thing in people, your body might clear this combo a bit more slowly, letting levels creep up.
What that could feel like is more of the usual side effects: low blood pressure, dizziness or lightheadedness, headache, flushing, a stuffy nose, or swelling. I want to be honest though, this is only theoretical right now, based on early lab work, not real patient data. Still, since this drug lowers blood pressure and treats a serious heart-lung condition, please check with your pharmacist or doctor before combining them.
