Nitrendipine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Here's the basic idea with these two together. Nitrendipine is a calcium channel blocker that your liver clears using an enzyme called CYP3A4. Lab studies suggest Terminalia arjuna may slow that same enzyme down. If that happens, your body clears nitrendipine more slowly, so more of it can build up. That could mean a stronger blood-pressure-lowering effect than expected, along with side effects like dizziness, flushing, headache, swollen ankles, or a faster heartbeat.

I want to be honest, though: this is only seen in test-tube research, not in real patients, so it's more of a theoretical concern than a proven problem. Still, since blood pressure is involved, it's worth a quick chat with your pharmacist or doctor before combining them.
