Siltuximab falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Here's some good news on this one. Siltuximab (Sylvant) is a large monoclonal antibody, not a typical pill that your liver's CYP3A4 enzymes break down. The interaction on file is about Terminalia possibly slowing CYP3A4, which would let certain drugs build up. But Siltuximab isn't really cleared that way, so this particular concern doesn't apply to it in the usual sense.

On top of that, the warning is theoretical and comes only from lab (test tube) studies, not from people taking these together. So I wouldn't lose sleep over it. Still, because Siltuximab treats a serious condition and Terminalia can affect the heart, please run any supplement past your doctor or pharmacist before adding it.

What Is Siltuximab Injection Used For?

Brand name: Sylvant

Sylvant (siltuximab) injection is approved to treat multicentric Castleman's disease (MCD) — a rare disorder involving abnormal lymph node growth driven by an overactive immune signal called IL-6. It is specifically approved for patients who test negative for both HIV and HHV-8 (human herpesvirus-8).

Sylvant has not been studied in patients with MCD who are HIV-positive or HHV-8-positive, because it does not block the form of IL-6 produced by those viruses.
