Amitriptyline, Perphenazine falls under SEIZURE THRESHOLD LOWERING DRUGS — the group Thuja interacts with.

Thuja products can contains significant amounts of thujone, a neurotoxin. Theoretically, patients taking drugs that lower the seizure threshold might be at greater risk of seizure if they also take thuja. Advise patients taking these drugs to avoid thuja products. Some drugs that lower the seizure threshold include anesthetics (propofol, others), antiarrhythmics (mexiletine), antibiotics (amphotericin, penicillin, cephalosporins, imipenem), antidepressants (bupropion, others), antihistamines (cyproheptadine, others), immunosuppressants (cyclosporine), narcotics (fentanyl, others), stimulants (methylphenidate), theophylline, and others.

This combo pill pairs amitriptyline (a tricyclic antidepressant) with perphenazine (an antipsychotic). Here's the thing: both of those medicines can already nudge your brain's seizure threshold a little lower, meaning your risk of a seizure goes up slightly. Thuja matters here because concentrated thuja oil contains thujone, a compound that can irritate the nervous system and, in large amounts, trigger seizures on its own.

Stack the two together and you could, in theory, raise that seizure risk further. I'll be honest, this concern is based on weak, anecdotal evidence, not solid studies, so don't panic if you've taken thuja before. Still, I'd steer clear of concentrated thuja products while on this medication, and check with your pharmacist or doctor before trying it.
