Dorzolamide, Timolol falls under SEIZURE THRESHOLD LOWERING DRUGS — the group Thuja interacts with.

Thuja products can contains significant amounts of thujone, a neurotoxin. Theoretically, patients taking drugs that lower the seizure threshold might be at greater risk of seizure if they also take thuja. Advise patients taking these drugs to avoid thuja products. Some drugs that lower the seizure threshold include anesthetics (propofol, others), antiarrhythmics (mexiletine), antibiotics (amphotericin, penicillin, cephalosporins, imipenem), antidepressants (bupropion, others), antihistamines (cyproheptadine, others), immunosuppressants (cyclosporine), narcotics (fentanyl, others), stimulants (methylphenidate), theophylline, and others.

Here's the gist with Cosopt (dorzolamide and timolol) and thuja. The concern with thuja is a compound called thujone, which in large or concentrated amounts can act as a nerve toxin and may lower the threshold for seizures. Cosopt is grouped with medicines that can nudge that seizure threshold down too, so the worry is that combining them could, in theory, make a seizure a bit more likely.

I want to be honest about how thin this evidence is. It's anecdotal and largely theoretical, not something well documented in real patients, especially for an eye drop where very little drug reaches your bloodstream. Still, since concentrated thuja oil can be genuinely toxic, I'd steer clear of it and check with your pharmacist or doctor before trying any thuja product.

What Is Dorzolamide and Timolol Ophthalmic Used For?

Brand name: Cosopt

Dorzolamide and timolol ophthalmic eye drops are used to reduce elevated intraocular pressure (pressure inside the eye) in people with open-angle glaucoma or ocular hypertension (higher-than-normal eye pressure without signs of glaucoma damage). It is specifically intended for patients who have not responded well enough to beta-blocker eye drops alone.

This combination is available as Cosopt, Cosopt PF, and Dorzolamide Hydrochloride and Timolol Maleate eye drops. Cosopt PF is the preservative-free formulation, which may be preferred for people sensitive to preservatives.
