Fosfomycin falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here's the gist for these two together. The concern isn't that fosfomycin (Monurol) changes how your thyroid extract works in any major way. It's the reverse situation that the interaction note really speaks to: oral medicines and thyroid hormone can compete for absorption in your gut if you swallow them at the same time. Fosfomycin is its own active antibiotic (it isn't a prodrug that needs activating), so the worry is simply timing.

In real life this is a minor issue. Fosfomycin is usually a single one-time packet mixed in water for a urinary infection, so it's easy to space apart from your thyroid product. To be safe, keep your thyroid extract on an empty stomach and ask your pharmacist about ideal timing so neither one loses its punch.

About Fosfomycin

What Is Fosfomycin Used For?

Brand name: Monurol

Fosfomycin is an antibiotic used to treat urinary tract infections (UTIs) caused by specific bacteria. The right form depends on how serious the infection is.

Oral forms (Fosfomycin Tromethamine and Monurol) are approved only for uncomplicated UTIs — also called acute cystitis — in women 18 and older, caused by E. coli or Enterococcus faecalis. They are not for kidney infections or abscesses around the kidney. Contepo (intravenous) is approved for adults 18 and older with complicated UTIs, including acute pyelonephritis (kidney infection), caused by susceptible E. coli or Klebsiella pneumoniae.
