Nortriptyline falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here the concern runs the other way from what folks often expect. It isn't that nortriptyline changes your thyroid extract. Instead, taking the two close together can reduce how well the thyroid hormone gets absorbed, so you may get less of it than you intended. With over-the-counter thyroid extract that already has unpredictable hormone amounts, that makes dosing even harder to pin down.

A simple fix is timing. Thyroid hormone absorbs best on an empty stomach, so spacing it well apart from your nortriptyline usually helps. Keep in mind nortriptyline itself can cause drowsiness, dry mouth, constipation, or a faster heartbeat, and untreated low thyroid can worsen low mood. Please loop in your pharmacist or doctor before changing anything.
