This combination pill already has two ingredients that can make you drowsy: codeine (an opioid pain reliever) and butalbital (a barbiturate that calms the nervous system). The concern here is that berberine, the active compound in tree turmeric, has shown mild sedative effects in animal studies. Stacked on top of this medication, it could theoretically add to the sleepiness, leaving you more groggy, dizzy, slowed in your thinking, or unsteady on your feet.

I want to be honest about the strength of this: it's based on lab and animal research, not human studies, so it's more of a caution than a proven problem. Still, since this pill already carries a real sedation risk, it's worth a quick chat with your pharmacist or doctor before adding tree turmeric, especially when driving or before bed.

Theoretically, tree turmeric might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro research and preliminary clinical evidence suggest that berberine, a constituent of tree turmeric, can inhibit CYP2D6.

Here's the tricky part with this combo pill. The codeine portion is actually a prodrug, meaning your body has to convert it into morphine using a liver enzyme called CYP2D6 before it can ease your pain. Berberine, the active compound in tree turmeric, may gently block that same enzyme. So instead of making codeine stronger, slowing this enzyme could leave you with less pain relief because less codeine gets turned into morphine.

Keep in mind this is mostly theoretical and based on lab work plus a few small studies, so it may not be dramatic for everyone. Still, if you notice your usual dose isn't working as well, or you're unsure, please check with your pharmacist or doctor before mixing the two.
