Here's the simple version. Apadaz (benzhydrocodone with acetaminophen) is a narcotic pain reliever that already slows down your brain and nervous system. That's why it can make you feel drowsy, foggy, or short of breath if your dose is high. Tree turmeric, thanks to its berberine, has shown calming or sedative effects in animal studies, so the worry is that stacking the two could pile on extra drowsiness.
To be honest, this concern is theoretical and comes from lab and animal research, not from people. So I wouldn't panic. Still, since the opioid here is the powerful part, play it safe: watch for added sleepiness or feeling unusually out of it, and check with your pharmacist or doctor before pairing them.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro research and preliminary clinical evidence suggest that berberine, a constituent of tree turmeric, can inhibit CYP2D6.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's something worth knowing about Apadaz. The benzhydrocodone part is actually a prodrug, meaning your body has to convert it into hydrocodone (the active painkiller) before it can work. That conversion leans on an enzyme called CYP2D6. Berberine, the main active ingredient in tree turmeric, may block that enzyme.
So in theory, taking concentrated berberine supplements alongside this medication could slow how much active painkiller you actually make, which might mean less pain relief than you expected. Keep in mind this is based on lab work and early human data, so it's a possible concern, not a sure thing. The acetaminophen side isn't really affected here. Before combining them, check in with your pharmacist or doctor so your pain stays well controlled.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro research and preliminary clinical evidence suggest that berberine, a constituent of tree turmeric, moderately inhibits CYP3A4.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the wrinkle with Apadaz (benzhydrocodone and acetaminophen): benzhydrocodone is a prodrug. Your body has to convert it into hydrocodone (the part that actually relieves pain) before it works. Tree turmeric, through its berberine content, can mildly slow down the CYP3A4 enzyme that helps process this medication. The catch is that hydrocodone leans more on a different enzyme (CYP2D6) for activation, so the real-world effect here is hard to predict.
To be safe, lean toward caution. If berberine nudges hydrocodone levels up, you could feel more drowsiness, dizziness, constipation, or slowed breathing. Don't start or stop anything on your own. Just loop in your pharmacist or doctor, especially since opioids leave little room for surprises.

What Is Benzhydrocodone and Acetaminophen Used For?
Brand name: Apadaz
Apadaz is indicated for the short-term management of acute pain — up to 14 days — that is severe enough to require an opioid and for which non-opioid treatments have not worked or are not expected to work. It combines benzhydrocodone (a prodrug that converts to the opioid hydrocodone) with acetaminophen.
Because of serious risks including addiction, abuse, and misuse, Apadaz is reserved for patients who have not tolerated or have not gotten adequate relief from alternative pain treatments. It should not be used for an extended period of time unless the pain remains severe enough to require an opioid and alternatives remain inadequate.
