Tree Turmeric and Chlorpheniramine Maleate, Dextromethorphan Hydrobromide

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Tree Turmeric interacts with whole categories of medication. Chlorpheniramine Maleate, Dextromethorphan Hydrobromide falls into 3 of those categories, so it can interact with Tree Turmeric in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

1 of 3

Why this applies to Chlorpheniramine Maleate, Dextromethorphan Hydrobromide

Chlorpheniramine Maleate, Dextromethorphan Hydrobromide falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Tree Turmeric interacts with.

Theoretically, tree turmeric might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro research and preliminary clinical evidence suggest that berberine, a constituent of tree turmeric, can inhibit CYP2D6.
Severity Moderate Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Both ingredients in this cough and cold combo lean on a liver enzyme called CYP2D6 to be cleared from your body. The berberine in tree turmeric may gently slow that enzyme down. In plain terms, your body could break down the dextromethorphan and chlorpheniramine a bit more slowly, letting them build up more than expected.

If that happens, you might feel extra drowsy, dizzy, or foggy, have a dry mouth, or in rarer cases feel restless or confused from too much dextromethorphan. Keep in mind this is mostly theoretical and based on lab and early human data, so it may not be a big deal for everyone. Still, if you take berberine regularly, it's worth a quick chat with your pharmacist before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 3

Chlorpheniramine Maleate, Dextromethorphan Hydrobromide also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, tree turmeric might increase the levels and clinical effects of drugs that are substrates of CYP3A4.
In vitro research and preliminary clinical evidence suggest that berberine, a constituent of tree turmeric, moderately inhibits CYP3A4.
Severity Moderate Likelihood Probable Evidence A

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

This cough and cold combo contains dextromethorphan and chlorpheniramine, both of which your liver helps clear (dextromethorphan partly through CYP3A4, along with CYP2D6). The berberine in tree turmeric can gently slow down CYP3A4. In plain terms, that means these ingredients could linger a bit longer in your system, nudging up their effects.

What might you notice? Possibly more drowsiness, dizziness, dry mouth, or a foggy, sedated feeling, and at higher dextromethorphan levels some people feel lightheaded or jittery. The evidence here is mostly theoretical and based on early studies, so for most people this isn't a big deal. Still, since both products already cause sleepiness, it's worth running it by your pharmacist before pairing them.

DEXTROMETHORPHAN (Robitussin DM, others)

3 of 3
Theoretically, tree turmeric might increase levels of dextromethorphan and potentially increase the risk of adverse effects including drowsiness, confusion, and irritability.
Preliminary clinical research suggests that berberine, a constituent of tree turmeric, can inhibit cytochrome P450 2D6 (CYP2D6) activity and reduce the metabolism of dextromethorphan.
Severity Moderate Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

This combo product contains dextromethorphan, the cough suppressant, and it relies on a liver enzyme called CYP2D6 to be cleared from your body. Berberine, the active piece of tree turmeric, can slow that enzyme down. When the enzyme is blocked, dextromethorphan can build up higher than usual, which may turn up its typical side effects like drowsiness, dizziness, confusion, or feeling restless and irritable.

Keep in mind this is mostly a theoretical concern backed by early, lower-quality research, so it is not a sure thing. Still, the chlorpheniramine part already makes you sleepy on its own, so the two together could leave you extra groggy. Concentrated berberine supplements are more likely to matter than tiny amounts in food. Before pairing them, just check with your pharmacist or doctor.

From our pharmacist’s overview on the Chlorpheniramine Maleate, Dextromethorphan Hydrobromide page

“Grapefruit is the interaction I would flag first for anyone taking chlorpheniramine with dextromethorphan (Coricidin HBP Cough & Cold). It blocks the enzyme that clears these ingredients, so levels and side effects can climb.”
