Macitentan, Tadalafil falls under ANTIHYPERTENSIVE DRUGS — the group Tree Turmeric interacts with.

Theoretically, taking tree turmeric along with antihypertensive drugs might have additive effects and increase the risk of hypotension.
Animal research suggests that berberine, a constituent of tree turmeric, can have hypotensive effects. Also, a meta-analysis of clinical research suggests that taking berberine in combination with amlodipine (Norvasc) can lower systolic and diastolic blood pressure when compared with taking amlodipine alone.

Opsynvi combines two blood-pressure-lowering medicines: macitentan (which relaxes blood vessels) and tadalafil (the same drug family as Cialis, which also widens vessels). So this combo already nudges your blood pressure downward on purpose. Tree turmeric, mostly because of its berberine, may have its own mild blood-pressure-lowering effect. Stacking them together could add up and drop your pressure a little too far.

What that might feel like: dizziness, lightheadedness, headache, flushing, or feeling faint when you stand up. To be fair, this warning leans mostly on lab and animal data, so it's more theoretical than proven in people taking Opsynvi. Culinary pinches are unlikely to matter, but concentrated berberine supplements are stronger. Please check with your pharmacist or doctor before adding it.

Opsynvi combines two medicines, macitentan and tadalafil, to treat pulmonary arterial hypertension (high blood pressure in the lungs). Berberine, the active piece of tree turmeric, may block a liver enzyme called CYP2C9 that helps clear these drugs. If that enzyme slows down, more of the medicine can linger in your body, which could make its effects and side effects stronger.

With this combo, that might mean more noticeable low blood pressure, dizziness, lightheadedness, headache, flushing, or swelling. Keep in mind this is mostly theoretical and based on early, lower-quality evidence, so it may not turn into a real problem for you. Still, before adding tree turmeric or berberine, please check with your pharmacist or doctor first.

Theoretically, tree turmeric might increase the levels and clinical effects of drugs that are substrates of CYP3A4.
In vitro research and preliminary clinical evidence suggest that berberine, a constituent of tree turmeric, moderately inhibits CYP3A4.

Opsynvi combines two medicines, macitentan and tadalafil, and both are cleared by a liver enzyme called CYP3A4. The berberine in tree turmeric can slow that enzyme down. If it does, these drugs may clear more slowly and build up in your body, which can mean stronger effects and more side effects.

For this combo, that could look like a bigger drop in blood pressure (feeling dizzy or lightheaded), headache, flushing, stuffy nose, or leg swelling. This concern is mostly theoretical and based on lab and early human data, so it may not cause trouble for everyone. Still, since blood pressure matters here, please check with your pharmacist or doctor before adding concentrated berberine supplements, and don't change your Opsynvi on your own.
