Macitentan, Tadalafil falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Turkey Tail Mushroom interacts with.
Laboratory research suggests that the PSP component of turkey tail mushroom dose-dependently inhibits CYP2C9. Theoretically, taking PSP with drugs metabolized by CYP2C9 might increase drug levels and the risk of adverse effects. However, this has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Opsynvi combines two blood pressure medicines for pulmonary hypertension: macitentan and tadalafil. Here's the idea behind this pairing. A component of turkey tail called PSP might slow down an enzyme (CYP2C9) that helps your body break these drugs down. If that enzyme works slower, the medicines could linger a bit longer and build up, which in theory could make side effects more likely, things like headache, flushing, low blood pressure, dizziness, a stuffy nose, or swelling in the legs.
I want to be honest about the strength of this, though. It comes from lab studies only, and it has never actually been seen in people. So it's more of a theoretical caution than a real-world red flag. Still, since these are heart and blood pressure medicines, it's worth a quick chat with your pharmacist or doctor before adding turkey tail.
