Devil's claw is generally well tolerated for short-term use in healthy adults, though it can cause digestive upset. The most common side effects are allergic skin reactions, diarrhea, and indigestion.

Gastrointestinal complaints show up in about 9% to 18% of people taking devil's claw extracts; diarrhea alone occurs in roughly 8% of users. Rarely, people have reported headache, tinnitus (ringing in the ears), loss of taste, dizziness, sleepiness, or trouble sleeping — though it's unclear if devil's claw caused these.

Very rarely, gastrointestinal bleeding has been reported. You should avoid devil's claw during pregnancy because it may affect the uterus and safety hasn't been established.
